Sleep apnea symptoms in women: why midlife changes the picture

Updated October 20, 2026 · 3 min read · Sources listed below

Short answer. In women, sleep apnea often shows up as tiredness, morning headaches and insomnia rather than the loud snoring people expect. The US National Heart, Lung, and Blood Institute lists fatigue, headache and insomnia as more common in women, and loud snoring as more common in men. Risk rises after menopause. If you wake unrefreshed, gasp at night or someone has seen your breathing stop, ask a clinician about a sleep study.

"But I don't even snore"

Sleep apnea means breathing repeatedly stops and starts during sleep. The picture most people have is a man snoring loudly. That picture misses many women.

The NHLBI notes that frequent loud snoring is more common in men, while fatigue, headache and insomnia are more common in women. A 2025 review in the journal Chest adds that women with sleep apnea are more likely to report insomnia and mood symptoms, and less likely to have loud snoring or breathing pauses that someone else notices.

Symptoms to know

At night (often noticed by a partner first):

In the day (you may notice these yourself):

Many of these overlap with perimenopause itself, which is one reason apnea can be missed. These signs are worth raising with a clinician, not a reason to self-diagnose.

Why women get missed

The Chest review describes sleep apnea as affecting up to 49% of men and 23% of women, yet it remains highly underdiagnosed. The authors point out that common screening questionnaires were built around symptoms that are more typical in men, such as loud snoring and observed pauses. If your main complaint is "I sleep badly and I'm exhausted", apnea may not be the first thing anyone checks.

What menopause changes

Hormone therapy is not a treatment for sleep apnea. Diagnosis and treatment go through a clinician and, usually, a sleep study.

Snoring that started around menopause

New or louder snoring in your late 40s or 50s is worth paying attention to, especially with daytime sleepiness.

  1. Ask your partner whether your breathing pauses or you gasp.
  2. Note how you feel on waking for two weeks: headache, dry mouth, unrefreshed. A printable sleep tracker makes this easy to bring to an appointment.
  3. Mention the pattern to your clinician.

When to see a clinician

Book an appointment if someone has seen your breathing stop, you wake gasping or choking, you feel sleepy while driving, or you wake with headaches on most days. A sleep study is how sleep apnea is diagnosed.

If you mostly struggle to fall or stay asleep, CBT-I is the first-line treatment for insomnia. Insomnia and apnea can exist together, so mention both.

Frequently asked questions

Can menopause cause sleep apnea?

Menopause is linked to higher risk. In the Wisconsin Sleep Cohort, postmenopausal women had higher odds of sleep-disordered breathing than premenopausal women, even after adjusting for age and body size. Ageing and weight also play a part.

Do women with sleep apnea always snore?

No. Loud snoring is more common in men. Women more often report fatigue, headaches and insomnia, which is one reason apnea is underdiagnosed in women.

Is it insomnia or sleep apnea?

It can be either, or both. Trouble falling or staying asleep points to insomnia; gasping, breathing pauses and unrefreshing sleep point to apnea. A clinician can tell them apart, sometimes with a sleep study.

Is snoring during menopause normal?

Sleep-disordered breathing becomes more likely after menopause, so snoring is worth mentioning to a clinician if it comes with daytime sleepiness, morning headaches or pauses in breathing.
